Social Worker

Our skilled nursing facility is seeking a Social Worker with a passion for working with elders , to fill a full -time position .

The Social Worker will assist in providing psychological, rehabilitation, and social support to our patients and families. To be a successful Social Worker, you should have excellent communication, interpersonal, and problem-solving skills. Ultimately, a top-notch candidate is compassionate and empathetic.

We would like to speak with a Social Worker who is skilled in communication, assessment and social work methods and techniques. You should be able to work effectively with a variety of disciplines in an individual and team setting. The successful candidate should have a working understanding of social systems along with the ability to implement pertinent state and federal regulations.

Don't forget...

• Experience in social work, admissions, and / or discharge planning in a nursing home or other long-term care setting is required.
• Master's degree in a social services or related field required.
• Social work license is required - LICSW, LCSW or LSW
• Knowledge of the Medicaid process is required.
