Certified Nursing Assistant (CNA) Job Description
Location : A New Well, Inc. St. Charles, Missouri
Position Type : Full-Time / Part-Time
Reports To : LPN, RN Supervisor & Program Director
Position Summary
The Certified Nursing Assistant (CNA) provides direct care, supervision, and support to individuals served by A New Well, Inc. in the Adult Day Program, Group Home, and / or community-based settings. The CNA ensures individuals health, safety, dignity, and independence while following all Missouri DHSS, DMH, Medicaid waiver, HIPAA, and A New Well, Inc. policies.
Essential Duties & Responsibilities
1. Direct Care & Support
- Assist individuals with Activities of Daily Living (ADLs), including bathing, grooming, dressing, toileting, and feeding.
- Provide mobility assistance, transfers, and safe ambulation.
- Support individuals with personal hygiene and incontinence care.
- Ensure safety and provide supervision throughout the program day.
- Promote independence and person-centered care.
2. Health Monitoring
Observe and report changes in health or behavior to the LPN / RN immediately.Take and record vitals (temperature, pulse, respirations, blood pressure) as trained.Monitor dietary needs, fluid intake, and special meal plans.Assist with non-medical tasks and support care plans.3. Documentation & Compliance
Complete daily progress notes, flow sheets, incident reports, and attendance accurately.Maintain confidentiality and follow HIPAA standards.Document services according to Missouri Medicaid waiver and DHSS requirements.Record ADL support, behavioral observations, and activities.4. Program Support
Assist individuals with learning, community outings, and skill-building activities.Help implement Behavior Support Plans, ISP goals, and day-to-day routines.Participate in team meetings when needed.Support transportation by assisting with loading / unloading vehicles safely (if assigned).5. Environment, Safety & Cleanliness
Maintain a clean, sanitary, and organized environment.Follow infection control, universal precautions, and agency safety procedures.Report hazards, maintenance concerns, and safety risks immediately.Assist with emergency response and drills.6. Professionalism & Conduct
Maintain a professional demeanor with individuals, coworkers, families, and guardians.Uphold A New Well's mission, values, and standards of care.Maintain required certifications and complete all mandatory trainings (CPR / First Aid, Abuse / Neglect, HIPAA, etc.).Qualifications
Active Missouri CNA certification (required).CPR / First Aid certification (or willingness to obtain).Experience in adult day programs, long-term care, group homes, or home health (preferred).Ability to lift 2540 lbs, assist with transfers, and perform physical care tasks.Strong communication, patience, and teamwork skills.Ability to accurately document services and follow care plans.Physical Requirements
Ability to bend, lift, push / pull, stand, and walk for extended periods.Ability to assist individuals with mobility and personal care.Ability to respond quickly in emergency situations.Schedule
MondayFriday, or based on program needs.Weekends or evenings may be required depending on group home staffing.Benefits
