Job Description
Job Title : LPN

License : NY LPN Licensure

Must-Have : Ability to provide care to geriatric residents
Description :
The LPN must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on the assigned unit.
The individual must demonstrate knowledge of the principles of growth and development across the lifespan.
Possess the ability to assess data reflective of the patient's status.
Interpret the appropriate information needed to identify each patient's requirements relative to age-specific needs.
Provide direct and indirect skilled nursing care for assigned residents.
Follow defined policies and procedures under the supervision of a Registered Nurse.
Maintain a collaborative relationship with other nursing staff, physicians, and ancillary staff.
Maintain an appropriate level of knowledge and clinical expertise through continuing education and in-service programs.
Collect relevant data regarding the resident's physical and psychosocial condition on an ongoing basis.
Communicate findings to the Registered Nurse and other members of the Interdisciplinary Team.
Contribute to the creation of a culturally relevant plan of resident care that reflects nursing diagnoses and interventions.
Promote a healthy, safe environment involving the resident / family and healthcare team in the process of care.
Recommend revisions to the plan of care after evaluating resident outcomes.
Indicate the need for referrals to ensure all aspects of the resident's care have been addressed.
Collaborate with the resident / family and members of the Interdisciplinary Team within the scope of nursing practice.
Assure customer satisfaction through effective communication and collaboration.
Participate in mentoring / orienting staff.
Consider factors related to safety, cost, and impact in planning the delivery of care.
Provide leadership in the professional practice setting under the direction of the Registered Nurse.
Demonstrate knowledge and competency in nursing practice.
Occupational exposure to bloodborne pathogens is anticipated.
Required tasks may involve potential contact with blood, body fluids, or tissues.
