• All adult patients " some w/ Medicare Advantage

• In Office " Outpatient position

• Monday through Friday 8:00am " 5:00pm

• No weekends

• Low patient panel

• Great Work/Life balance

• Comfortable with use of Electronic Medical Records (EcW)

• Detect and manage chronic diseases

• Develop patient strategy to reduce hospitalization and ER visits

• Collaborate with Case Managers and Specialty Providers

• Educate patients on the value of compliance

Requirements:

• BC/BE Family Medicine Physician

• Active Florida medical license

• 2 years Primary Care experience

• 1 year Managed Care experience
