PRN Home Health & Therapy is seeking a compassionate and skilled Registered Nurse to join our growing team in Madison. As a home health clinician, youll provide care to patients in Assisted Living Facilities (ALFs) and private residencesbringing quality healthcare directly to those who need it most.

You are an RN who

A licensed RN (or eligible for licensure in Wisconsin)

Passionate about building meaningful connections with patients

Comfortable working independently while supported by a collaborative team

Skilled in both written and verbal communication

Key Responsibilities

Serve as Case Manager, coordinating comprehensive patient care

Conduct and document thorough assessments; develop individualized treatment plans

Collaborate with patients, families, and clinical staff to optimize outcomes

Maintain clear and concise documentation, including discharge summaries

Educate patients and caregivers on safe discharge planning in coordination with the clinical team
