Must Have Qualifications: Qualifications

• Bachelor's Degree in Human Services, Education or a related area
• Licensed Practical Nurse (LPN) NYS license
• Two (2) years' work experience in benefits and entitlements with extensive knowledge of community resources
• Experience with HIV/AIDS afflicted individuals, chronic mental illness, substance abuse individuals and their families a plus
• Bilingual skills in English/Spanish a plus
• Knowledge and hands-on experience in providing direct services related to the treatment and recovery of persons with HIV/AIDS, substance abuse, serious mental illness, and homelessness
• Excellent oral and written communication skills;proficiency with MS Office and Google product suites
• Ability to perform and manage in a team approach;excellent interpersonal skills
• Ability to handle complex challenges and balance multiple demands on time and attention

Day-to-Day Responsibilities:
Conduct needs assessments

Establishing a network of community resources to facilitate wellness activities

Recommend appropriate activities, lead and co-lead activity groups and encourage clients' participation in community events

Provide wellness-related counseling and monitor clients' progress with their goals

Work collaboratively in an inter-disciplinary team to ensure comprehensive, client-centered services are delivered and documented
