Job Description:

Responsibilities:

• Conduct comprehensive assessments of patients' physical, mental, and emotional health in their home environment.
• Develop and implement individualized care plans in collaboration with physicians and other healthcare providers, ensuring patient needs are met.
• Provide treatments, administer medications, and manage wound care as prescribed by the physician.
• Teach patients and their families about managing chronic conditions, medication regimens, and post-surgical care to promote self-care and recovery.
• Track patient progress, monitor for complications, and report any significant changes in the patient's condition to the physician or healthcare team.

Qualifications:

• MD Registered Nurse License
• Basic Life Support (BLS), Advanced Cardiovascular Life Support (ACLS)
• Minimum 1-2 years of nursing experience in a hospital, clinic, or other healthcare settings, preferably in areas like medical-surgical, geriatrics, or community health
