Essential Duties and Responsibilities:

• Cultivate safe and supportive relationshl supports via natural supports
• Assist individuals with developing socialization, meal preparation, and personal hygiene skills.
• Transport individuals to medical appointments, shopping, and recreation activities.
• Assist individuals to identify and facilitate volunteer opportunities or paid employment.
• Complete training based on the needs of the individuals
• Complete in-house documentation

Requirements:

• High school diploma or GED
• Negative TB skin test or chest x-ray and physical will be required
• Background check will be required
• Valid drivers' license with an acceptable driving record required.
• Current CPR Certification a plus
• Med Trained a plus
