Position Overview : Nurse Care Manager (Indiana Expansion)

This is a unique and exciting opportunity for a nurse who is motivated to help launch our services in a new state. The role will begin on a consulting-style, part-time basis , with a few hours per month of virtual work while we finalize the Indiana office.

Once the office opens, this position will transition into a more traditional Care Manager role with responsibilities that include :

• Conducting in-home assessments
• Developing individualized care plans
• Building strong relationships with patients, families, and medical teams
• Collaborating with our HR, Scheduling, and Care Manager teams
• Providing ongoing supervisory visits and client support
• Acting as a patient advocate to ensure safe, high-quality care
• Promoting education, independence, and self-management for clients
