Key Responsibilities
Manage change payor workflow for assigned markets
Coordinate new referrals into SPARC for Change Payor and Joint Venture EMR transition
Provide support for Clinical Field Staff Liaison workflows as needed
Required Qualifications, Training, and Education
Registered Nurse (RN) or Licensed Practical Nurse (LPN)
1+ year of Home Health Experience required
Maintains active nursing license in all states where services are provided
Experience with entering referrals into SPARC preferred
