Join our team in New York, NY as a Wellness Coach, where you'll support a high-risk population in a supportive housing facility. This temp-to-perm role offers the opportunity to make a direct impact in the community.
About the Opportunity:
- Onsite
- Shift: Day
- Schedule: Monday to Friday
- Hours: 9 am - 5 pmResponsibilities:
- Providing case management to assigned caseload
- Developing and monitoring individual support plans
- Assisting with problem-solving and crisis intervention
- Accompanying clients to necessary appointments
- Advocating for client needs and encouraging self-advocacy
Qualifications:
- 3 years of experience working with the behavioral health population
- Bachelor's degree preferred
- Proficiency in Microsoft Word, Excel, PowerPoint, and Windows
- Strong case management skills
- Experience with substance use and mental health
- Bilingual English/Spanish preferred
- Empathetic and team-oriented
- Ability to manage multiple tasks efficiently
- Strong communication skills
