[job_card.job_description]Act as the main point of contact and support resource for individuals navigating benefit programs Communicate with healthcare partners, internal teams, and outside vendors to move cases forward Help gather necessary documentation and verify program eligibility Keep accurate records and follow up on open tasks to ensure nothing falls through the cracks Resolve questions and troubleshoot issues with professionalism and empathy Support access to financial assistance by researching available resources Contribute to a smooth experience for each member while meeting internal service expectations 2-3 years of customer service experience; healthcare, PBM, or pharma background is a plus Strong written and verbal communication skills Empathy driven and professional approach to patient communication Organized, detail oriented, and able to manage multiple open cases Proficient in using CRM systems and documentation tools Pharmacy exposure in a previous role.

Care Coordinator :
