• RTO will not be approved in the first week of assignment or holidays.

• If interested in "any" location, please indlude what states (I.E. Any AZ, Any CO, Any state, etc.)
• All shifts the traveler is open to working on should be listed on the submission.
• All departments the traveler is open to working must be checked on the cover page.
• All candidates submitted should be preclosed and ready to accept.

Requirement description :

• Candidates with an AZ home address on their drivers license will not be considered - please include a copy in submission
• 24 months minimum of current acute Labor and Delivery surgical tech experience - REQUIRED
• Certifications must be American Heart Association OR American Red Cross, active upon submission and INCLUDED IN SUBMISSION - REQUIRED
• No License Hits - REQUIRED
• Banner will not approve RTO within the first week or holidays. Can not exceed 7 days.
• Flu vaccination - Banner declinations can be applied for (reach out for forms) **Please add vaccination status in the submission notes**

CERTIFICATION REQUIREMENTS :

• BLS (AHA)
• NBSTSA

ADDITIONAL LICENSE REQUIREMENTS : Must have active NBSTSA at time of submission
Weekend Requirements : Weekend schedule to be determined by facility
On Call Requirements : On call schedule to be determined by facility
