Position Overview

TruHealth is looking for a skilled Nurse Practitioner (NP) or Physician Assistant (PA) with a focus in primary care or internal medicine, particularly in geriatrics. You will play a crucial role in improving the health outcomes for residents in long-term care by collaborating with a dedicated team of healthcare providers and case managers. Our providers primarily work in nursing homes or assisted living facilities, delivering personalized healthcare to residents enrolled in specialized Medicare Advantage plans. Your role will focus on improving healthcare quality and preventing avoidable hospitalizations.

As part of our advanced practice team, you will utilize your independent judgment to treat patients with both acute and chronic conditions, collaborating with a supervising physician when necessary. You will work closely with our healthcare facilities and health plan leadership to implement our national care model, adhering strictly to federal and state regulations.

Key Responsibilities :

• Assess patients' medical and mental health needs, developing a comprehensive care plan that proactively manages their conditions and removes barriers to care.
• Ensure compliance with local, state, and federal regulations related to the clinical services provided.
• Prescribe medications as necessary and arrange for lab tests, diagnostic procedures, and consultations.
• Monitor patients' adherence to treatment plans and adjust those plans as needed.
• Collaborate with RN case managers as an integral part of an integrated care team.

Qualifications :

• Active FNP, AGNP, AHACNP, or PA license is required.
• Graduate degree from an accredited APRN or PA program.
• A minimum of 3 years of clinical nursing experience or rehab experience in geriatric populations is essential.
• Proficient in using Electronic Health Records and strong command of Microsoft Office applications including Word, Outlook, and Excel.

Additional Responsibilities :

• May involve providing training and guidance to facility staff.
