The Social Worker position provides psychosocial support to residents and families while assisting with care planning, discharge coordination, and advocacy. Social Workers help ensure residents’ emotional and social needs are met.

Conduct psychosocial assessments and care planning
Support residents and families with needs and resources
Coordinate discharge planning
Advocate for resident rights and quality of life
Maintain accurate documentation and compliance
Social Worker Compensation & Schedule
Employment Type: Full-Time / Part-Time / Per Diem
Pay Range: $75,000/year - $85,000/year
Social Worker Qualifications & Skills
Social work experience preferred
Long-term care experience a plus
Strong communication and organizational skills
