Responsibilities

  • Conduct detailed assessments of patients and their families.
  • Develop and manage individualized care plans.
  • Coordinate with interdisciplinary teams to provide holistic care.
  • Regularly review and update care plans based on patient needs.
  • Provide emotional support and education to families and caregivers.

    • Requirements

  • Certification : Licensed Registered Nurse with no restrictions.
  • Experience : Minimum 2 years in hospice care.
  • Skills :
