HOME HEALTH RN | DETAILS AND COMPENSATION:

• Location: Carmel IN – 100% On-site, home health settings
• Payrate: $55.29/hr + mileage
• Required Availability: Full-Time | Monday–Friday, 8:00 AM – 5:00 PM (approximately 4–6 visits per day)
• • On-Call Requirement: One weeknight per week and one weekend per month
    Hours: 40 hours not guaranteed

HOME HEALTH RN | SUMMARY AND HIGHLIGHTS:

The Home Health RN willprovide high-quality home health services in a community-based setting. This role focuses on patient assessments, care plan implementation, case management, and coordination with interdisciplinary healthcare teams. The RN will support patients with chronic, acute, or long-term healthcare needs while ensuring compliance with clinical standards and documentation requirements.

• This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
• Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance
• A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

HOME HEALTH RN | RESPONSIBILITIES:

• Conduct comprehensive patient assessments, including OASIS assessments
• Develop, implement, and evaluate individualized care plans
• Provide skilled nursing care in a home health setting
• Coordinate care with physicians, caregivers, and interdisciplinary teams
• Monitor patient conditions and report changes to physicians promptly
• Ensure accurate and timely clinical documentation
• Manage medication administration, storage, monitoring, and disposal per policy
• Supervise LPNs and support staff as needed
• Assist with discharge planning and coordination during hospitalizations
• Provide training and annual recertification on health and safety topics
• Maintain compliance with infection control and safety standards

HOME HEALTH RN | REQUIREMENTS:

• Education: High School Diploma or GED (verifiable)
• Attendance: Commitment to mandatory attendance during the first 90 days.
• Active Indiana RN License or Compact State License
• Home health nursing experience required
• Proficiency with OASIS assessments
• Strong patient relationship and communication skills
• High-level clinical and assessment skills
• Comfortable traveling between patient homes (approximately 4–6 visits per day)
• Technologically proficient (experience with HCHB strongly preferred, but not required)
