Weekend Home Health Registered Nurse RN

Salary: $50 - $55/hourly

Valley Regional Home Health is currently seeking apart-time or full-timeRegistered Nurse who would be able to work Saturdays and Sundays after training.Applicants must be able and willing to train Monday through Friday for at least one month.Training hours are generally 8:00am-4:30pm, but once the employee is independent, RNs can adjust their working days, start times, and end times to provide patient care in the home environment. Our home health RNs see between 3-5 patients per day and drive between patient visits.

Patient Care

• Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es). Ability to read documentation on patient status as well as any orders that the patient PCP may have provided.
• Regularly re-evaluates patient nursing needs and completes necessary charting within the required timeframe.
• Initiates the plan of care and makes necessary revisions as patient status and needs change; uses health assessment data to determine nursing diagnosis.
• Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.
• Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications, treatments, IV ABX, etc. as prescribed by the physician.
• Counsels the patient and family in meeting nursing and related needs, provides education on plan of care
• Provides health care instructions to the patient as appropriate per assessment and plan of care.
• Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.

Position Qualifications

• Registered nurse with current licensure to practice professional nursing in the state.
• Graduate of National League for Nursing accredited school of nursing.
• Maintains a current CPR certification.
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order - as well as willing to drive around the job posting area.
• Demonstrates excellent observation, problem solving, verbal and written communication skills; ability to work well with others and communicate time-sensitive information effectively.
• Shows ability to organize and prioritize workload independently home health employees are typically independent in the field.
• One year of home health experience preferred, but not required.
