Home Health Social Worker MSW, SSW, or LCSW

Required Location : Salt Lake Valley, UT Pay : $30-35 / hour Job

Overview : We are seeking a compassionate and dedicated Social

Worker to provide support to the geriatric community through Home

Health. The ideal candidate will have a strong background in

providing end of life support, offering community resources, and

discussing care goals with patients. This position will be

full-time in the Salt Lake City area. You are paid for drive time

and mileage! Responsibilities : Communicate effectively with the

internal team and those offering patient support Organize schedule

in an efficient manner to travel to assigned visits Provide helpful

resources and discuss care goals with elderly patients Navigate end

of life issues that require support and attention Offer crisis

management support during difficult situations Qualifications : MSW

required Valid driver's license Experience with geriatric community

preferred, not required This position offers the opportunity to

make a meaningful impact on those nearing the end of their life and
