LPN Nurse Navigator

• Make up to 20 outbound patient calls daily
• Conduct assessments within your LPN scope to understand each patient’s unique needs
• Create and collaborate on personalized care plans
• Serve as the communication bridge between patients, families, and providers
• Educate patients on their conditions, treatment options, and preventive care
• Track progress and adjust care plans as needed
• Document interactions and work within multiple healthcare systems and tools

• Active LPN license
• 2+ years of hands‑on patient care experience
• A HIPAA‑compliant home office with secure high‑speed internet
• Experience in patient care coordination or case management
• Strong clinical judgment and a heart for patient engagement
• The ability to work reliably Monday–Friday without shifting schedules

• Care coordination or case management certification
• Experience with diverse patient populations
• Comfort with various EHR systems and healthcare software
