ACE Home Health and Hospice is locally owned and operated agency. We have offices based in Orinda and San Jose, CA and we are looking for the right persons to join our team. We are growing and are seeking talented and compassionate professionals that want to make a difference in the lives of their patients. Positions can start as per diem and could quickly grow into full time employment.

Job Description:
Registered Nurse - Home Health
Responsible for the assessment and plan of care of home health patients in accordance with the RN scope of practice and in compliance with CMS guidelines and regulations for home health care. Responsible for creating the OASIS/start of care documentation, re-visits following start of care, care coordination and managing a team of therapists to ensure patients meet their home health goals, and can be discharge back to the community with emphasis on an excellent patient experience.
