As a General Practice Physician (PCP), you'll play a pivotal role in providing quality care in our ambulatory health care centers, as well as in diverse settings including acute care, nursing homes, and even patients' homes. Your responsibilities will encompass :

• Conducting comprehensive assessments including medical history, physical exams, diagnosis, and treatment plans.
• Performing geriatric assessments and developing individualized care plans.
• Providing education and managing specialty referrals and case management.
• Maintaining clear and thorough documentation in line with ChenMed's standards.
• Collaborating with healthcare teams and participating in clinical rounds.

You'll be empowered to make independent clinical decisions while ensuring compliance with quality care standards. Your ability to work cooperatively with multidisciplinary teams will help shape superior patient outcomes. In this role, you'll lead initiatives that enhance patient care, advocating for optimal resources and educating families on treatment goals.
