Job Description

We’re Expanding – Come Grow With Us!


Our long-term care group is excited to welcome a dedicated Primary Care Nurse Practitioner to our team in Rock Island, IL. If you’re passionate about caring for seniors and want to work where your skills are valued, we’d love to meet you!


Key Duties:

  • Full primary care services—consults, exams, prescriptions, and follow-ups
  • Acute issue management & diagnosis formulation
  • Work collaboratively with facility teams to maintain care standards


Requirements:

  • Active Illinois NP license & Board Certification
  • Long-term care experience preferred


Schedule:

  • Monday–Friday
  • No on call and no weekends!


What We Offer:

  • $115,000–$150,000 annual salary
