Key Responsibilities:

Perform thorough patient assessments and screenings across all age groups.
Diagnose and manage acute and chronic illnesses.
Develop individualized treatment plans and monitor patient progress.
Prescribe medications and treatments according to established protocols.
Educate patients and families on health promotion and disease prevention.
Collaborate closely with our interdisciplinary team to ensure comprehensive care.
Maintain accurate and detailed electronic health records.

Qualifications:

Current and unrestricted NM state Nurse Practitioner license.
National certification as a Family Nurse Practitioner (FNP).
Proven experience in family medicine or primary care.
Excellent communication skills and a patient-centered approach.
