United Health Care Services is seeking a reliable and compassionate Transportation Provider to join our State Medicaid Supported Community Connection (SCC) program. The primary responsibility of this role is to transport program participants to and from various community activities, appointments, and services. The SCC program supports individuals with disabilities in engaging with their communities and improving their quality of life.

Key Responsibilities :

  • Safely transport SCC participants to scheduled activities, appointments, and other destinations.
  • Assist participants in and out of the vehicle as needed.
  • Ensure the safety and comfort of all passengers during transport.
  • Maintain a clean and well-functioning vehicle.
  • Communicate effectively with participants, caregivers, and program staff regarding transportation schedules and any potential delays or changes.
  • Keep accurate transportation logs and records in compliance with Medicaid requirements.
  • Respond to any transportation-related emergencies or issues in a calm and professional manner.
  • Mitigate and manage extreme behaviors during transportation and SCC activities in accordance with program protocols, ensuring the safety and well-being of all participants.

Qualifications :

  • Valid driver’s license with a clean driving record.
  • Prior experience working with individuals with disabilities is preferred.
  • Ability to operate a vehicle safely and responsibly.
  • Must be dependable, punctual, and have a strong sense of responsibility.
  • Good communication and interpersonal skills.
  • Ability to follow directions and adapt to schedule changes.
  • Experience or training in de-escalating challenging behaviors and maintaining a calm environment during activities and transit.

    • Requirements :

  • Must pass a background check.
  • Must have reliable transportation (if using personal vehicle, must provide proof of insurance).
  • Ability to lift and assist participants as necessary.

    • Schedule :

  • Flexible hours, with the potential for full-time or part-time positions.

    • Compensation :

  • Competitive pay based on experience.
  • Gas reimbursement (if applicable).
