A company is looking for a Care Coordinator - Clinical.
Key Responsibilities
Receive and document incoming calls from members regarding benefits and care coordination
Manage an assigned caseload for short-term care coordination and health navigation
Perform outreach calls to confirm services and support communication within the interdisciplinary care team
Required Qualifications
Certified Medical Assistant (CMA), Certified Nursing Assistant (CNA), Registered Medical Assistant (RMA), or relevant medical training / experience
High school diploma or GED
Experience in a fast-paced environment managing multiple priorities
Bilingual in Spanish or Mandarin / Cantonese is preferred
Prior work experience in a healthcare environment is preferred
