Home Health Physical Therapist - Per Visit | Harrisburg, PA

We are seeking a dedicated and compassionate Home Health Physical Therapist (PT) to join our dynamic healthcare team in Harrisburg, Pennsylvania. In this rewarding per-visit position, you will play a crucial role in enhancing the quality of life for patients by providing personalized physical therapy services in the comfort and convenience of their homes. Job Responsibilities

  • Conduct comprehensive home-based physical therapy assessments to evaluate patient mobility, strength, balance, and functional limitations
  • Develop and implement individualized physical therapy treatment plans focused on improving patient mobility, reducing pain, and enhancing overall physical function
  • Deliver therapeutic exercises, manual therapy techniques, and evidence-based interventions tailored to each patient's specific rehabilitation goals
  • Provide patient and family education on home exercise programs, fall prevention strategies, and adaptive equipment use to promote independence
  • Collaborate with physicians, nurses, occupational therapists, and other healthcare professionals to ensure coordinated, holistic patient care
  • Maintain accurate and timely clinical documentation in compliance with state regulations and Medicare guidelines
  • Monitor patient progress, reassess treatment effectiveness, and modify care plans as needed to achieve optimal rehabilitation outcomes
  • Participate in interdisciplinary team meetings and contribute to quality improvement initiatives

Job Requirements

  • Current, unrestricted Physical Therapist license in the state of Pennsylvania
  • Graduate of an accredited Physical Therapy program (DPT preferred)
  • Home health physical therapy experience strongly preferred; new graduates with clinical rotations in home health considered
  • Valid driver's license, reliable transportation, and current auto insurance
  • Strong clinical assessment and critical thinking skills
  • Excellent interpersonal and communication abilities to effectively interact with patients, families, and interdisciplinary teams
  • Ability to work independently, manage time efficiently, and maintain a flexible schedule
  • Proficiency with electronic medical records (EMR) and documentation systems
  • CPR certification required
  • Commitment to delivering high-quality, evidence-based, patient-centered care
