[job_card.job_description]Provide therapeutic interventions under the direction and supervision of a licensed Physical Therapist. Assist patients with strengthening, mobility training, gait training, and functional exercises. Support patient progress toward individualized treatment goals and document daily sessions per facility standards. Communicate effectively with the supervising PT, interdisciplinary team, and facility staff regarding patient progress and needs. Maintain accurate and timely documentation within the facility’s EMR system. Follow established care plans and ensure safe, appropriate use of therapy equipment. Float between assigned facilities (within walking distance) when required. Active Physical Therapist Assistant (PTA) license in the state of California. Minimum of 2 years of experience, preferably in a Skilled Nursing Facility (SNF). Current BLS certification. Strong communication, time management, and patient-centered care skills. Ability to work both independently and collaboratively in a multidisciplinary environment. Full-time schedule, 40 hours per week. Monday–Friday preferred; Tuesday–Saturday schedule also acceptable. No holiday coverage required. Skilled Nursing Facility (SNF) environment. Medical benefits Referral bonus Relocation bonus

Job Summary :
