Arizona Licensed High Risk RN

A company is looking for a High Risk RN Care Coordinator.

Key Responsibilities

Participate in daily huddles and coordinate patient care with internal and external teams

Create patient-centered action plans and manage transitions of care to various facilities

Respond to patient calls and address care coordination tasks assigned by team members

Required Qualifications

AZ Licensed Registered Nurse (RN) with ability to obtain licensure in other states

5+ years of RN experience, including at least 1 year in care coordination or case management

Experience in complex care settings, preferably with senior health or home-based care

Advanced knowledge of care management principles and chronic disease education

Demonstrated clinical aptitude and ability to navigate health insurance policies
