Description

• Conduct patient evaluations in inpatient or outpatient settings, crafting personalized treatment plans tailored to individual needs
• Implement clinical interventions that focus on improving mobility, strength, balance, and overall functional ability
• Collaborate with a diverse team of healthcare professionals, including nurses, physicians, case managers, and other rehabilitation specialists to enhance patient outcomes
• Provide education to patients and their families regarding therapeutic goals, discharge processes, and home exercise protocols
• Maintain thorough documentation of patient evaluations, treatments, and progress in alignment with facility and regulatory standards
• Actively participate in interdisciplinary rounds and team conferences to ensure a cohesive approach to patient care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• Active state licensure as a Physical Therapist
• CPR certification is preferred
• Additional specialized certifications (e.g., in geriatrics or neurology) are advantageous but not mandatory

Skills

• Prior experience in a clinical setting is strongly preferred
