Job Description

A Long Beach based clinic is searching for a Nurse Practitioner who is passionate about improving individual and community health. An ideal candidate has experience conducting Annual Wellness Visits. This NP should be versatile as it requires home and telehealth visits. The NP would see 10 patients per day telehealth and 5-6 in-home visits.

Required Qualifications

• Current California Nurse Practitioner license
• Nurse Practitioner Certification from either the ANCC or AANP
• Current DEA licensure
• Active CPR Certification
• 1-2 years of NP experience (home care and geriatrics are a plus)
• Experience conducting Annual Wellness Visits
• Spanish (preferred)

Responsibilities

• Conduct annual wellness visits daily for primary care management of patient caseload
• Manage both acute and chronic conditions through direct intervention like medication prescription and patient education
• Consultation with collaborating Physician as appropriate
• Complete documentation of patient encounters, funding sources, and billing according to policy
