A company is looking for a Lead Case Manager, Bilingual (Spanish).
Key Responsibilities
Serve as the primary point of contact for members and their support teams, providing care management and coordination
Assess member needs and develop comprehensive Care Management Plans with input from multidisciplinary teams
Advocate for members with healthcare professionals and connect them to necessary social services and supports
Required Qualifications
Fluency in Spanish is required
Bachelor's degree preferred or 2 years of case management experience
Ability to work with diverse populations and in various environments
Current California driver's license and acceptable driving record
Knowledge of community resources and familiarity with trauma-informed care practices
