1) Conduct mental health assessments and assist in the development and implementation of comprehensive service plans that identify mutually agreed upon long and short-term mental health related goals.
2) Provide mental health services and referrals;assistance with adherence to medication regiments;activities for daily living;nutritional services;recreation programs;assistance in employment assessments, training;prevention with positives;and other necessary services to clients to increase their capacity for independent living. Also, monitor and ensure that clients are linked to needed services.
3) Provides, coordinates and collaborates with providers
5) Make home visits and maintains face to face contact as specified in the service plan(s)
6) Responsible to develop and implement weekly groups including the Wellness Self-Management model.
7) Responsible for the timely completion of all required documentation including, comprehensive assessments, reassessments, progress notes of all contacts with clients and / or collaterals.
8) Participate in case conferences, training, workshops and seminars as well as meetings with peers and supervisors as required.
9) Assist in the planning, organizing program recreational activities.
10) Participates in weekly supervision. Work with HIV and Aids patients
