Licensed Social Worker Case Manager

A company is looking for a Case Manager Licensed Social Worker.

Key Responsibilities

Conduct thorough holistic assessments addressing various patient needs

Develop and implement treatment plans for standard and catastrophic cases

Advocate for patients while coordinating care and managing costs

Required Qualifications, Training, and Education

Graduation from an accredited college with a degree in Social Work

License in Social Work is required

Minimum of 2 years of acute healthcare experience, including 2 years in case management

Basic computer literacy and proficiency in Microsoft Office applications

Knowledge of medical claims and coding standards (ICD-10, CPT, HCPCS)
