[job_card.job_description]Providing occupational therapy to patients struggling with daily tasks due to illness or injury Collaborating within a home health team, including RNs, LPNs, CNAs, HHAs, PTs, COTAs, and SLPs, to develop and follow patient-centered care plans Assessing patient needs, observing deficits, establishing therapy goals, and documenting progress Assisting patients with exercises to enhance fine motor skills and coordination Recommending adaptive equipment such as grab bars and shower chairs for added support Evaluating fall risks and implementing strategies to enhance home safety Educating patients and families about care plans, exercises, goals, and self-care practices Degree from an accredited Occupational Therapy Program and an active OT license in Colorado At least two years of relevant experience as an occupational therapist in the last five years preferred Current CPR certification (course required) Familiarity with state and federal home health regulations Strong clinical judgment, interpersonal skills, resourcefulness, and compassion No Covid vaccination requirements
