Licensed Practical Nurse (LPN) - Home Care -    Enjoy independence, autonomy, and flexibility in the field to visit patients one on one in their home environment.

Hours : Full-time | Days, Monday - Friday | There will be a weekend and holiday rotation. Dependent upon staffing levels, work approximately one weekend every 4-6 weeks and approximately 2 holiday's per year.

Territory : The LPN will have a general territory within our service area where they will provide one on one in-home care to the patients.  The office is in Roseville, MN and the general territory for this position would primarily be in the southeast section of the Twin Cities metro area.

  • A few must-haves for this position :

  • Current license as a Licensed Practical Nurse (LPN) in the State of MN.   License is active and in good standing.
  • 2 years' experience as an LPN preferred.
  • Able to travel within geographic area serviced by the office. Valid driver's license, reliable transportation, and proof of auto insurance required.
  • CPR Certification
  • Some functions of what you would do as an LPN :

  • Participate in ongoing interdisciplinary assessment of the patient.
  • Participate in the development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s).
  • Provides services that are ordered by the physician as indicated in the plan of care.
  • Recognizes and reports changes in the patient’s condition to the RN supervisor.
  • Counsels and educates the patient and family in meeting healthcare related needs.
  • Completes timely and accurate clinical notes including addressing patient’s progress.
