[job_card.job_description]Conduct thorough assessments of patients' physical needs and develop individualized care plans. Implement therapeutic interventions, including exercises and modalities, tailored to patients' conditions. Monitor patient progress and make necessary adjustments to treatment plans. Educate patients and their families on home exercise programs and injury prevention strategies. Maintain accurate documentation in compliance with regulatory standards, including OASIS assessments. Coordinate with other healthcare professionals to ensure holistic patient care. Minimum of 1 year of skilled home health experience required. BLS certification is required. Experience with Epic charting system preferred. Valid state license to practice as a Physical Therapist. Prior experience in Acute Care, Rehabilitation Hospitals, or Skilled Nursing Facilities is a plus. Ability to use adaptive devices and modalities, including AROM, PROM, E-stim, and TENS preferred. Dementia Parkinson's Disease ALS Multiple Sclerosis Orthopedic post-operative conditions Post-stroke rehabilitation Home health setting; the traveler is expected to have their own transportation. Travel radius is generally within 60 miles, with minimal drive time. COVID-19 vaccination (primary series plus one booster) is required. Free parking and mileage reimbursement available at IRS standard rates. Business casual attire is acceptable; scrubs permitted.
