Implementing residents’ personal care plans, focusing on residents’ strengths, preferences, and preferred routines, while providing a sense of comfort, companionship, and belonging. Working with the interdisciplinary clinical team to ensure the needs of our residents are being addressed throughout their care. Minimum 1 years’ experience in working with the older adult population related to the therapies is preferred. Current CPR Certification. Current state license (for the state in which the community provides service) in physical therapy, where applicable.

All of our residents live within our award-winning campus, creating an environment where you can be part of their journey throughout their continuum of care.
- This is a 24 hour a week part-time position in our Home Health Department. Home Health experience is required
