SUMMARY
Join our dedicated team as a Nurse Practitioner specializing in End-Stage Kidney Disease (ESKD) and Chronic Kidney Disease (CKD). This role provides an exciting opportunity to make a significant impact on patients' lives while working collaboratively within a supportive healthcare environment.
Essential Duties and Responsibilities: The primary duties include, but are not limited to:
- Conducting ESKD rounding with guidance from practice nephrologists, including both limited and comprehensive visits.
- Managing holistic patient care for ESKD patients, focusing on patient activation, addressing mental health, preventing hospitalization, and overseeing co-morbid medical issues.
- Providing ESKD patient education to promote understanding of their condition and care.
- Conducting comprehensive disease management annual visits for ESKD patients.
- Collaborating with nephrologists, dialysis center teams, hospital discharge coordinators, primary care physicians, and specialists as required.
- Providing CKD clinical care and educating patients on kidney health.
- Performing other duties as assigned.
Key Responsibilities: In this role, you will:
- Perform thorough physical examinations to assess health and diagnose illnesses.
- Order and interpret diagnostic tests, including x-rays and lab tests.
- Develop, initiate, and adjust treatment plans as necessary.
- Prescribe medications to support patient health.
- Maintain accurate electronic medical records and document patient information.
- Educate patients and families on health issues, treatment options, and home care, while promoting preventive health behaviors.
- Request consultations with healthcare providers as needed and collaborate closely with the Kidneylink RN Care Manager.
- Create a welcoming atmosphere by actively promoting GUEST customer service standards and fostering relationships throughout the organization.
- Encourage teamwork and participate in a collaborative work environment, adhering to all company policies and procedures.
- Maintain reliable and regular attendance.

Qualifications/Requirements: To succeed in this role, candidates must meet the following requirements:
- Current license to practice as an Advanced Practitioner in the relevant state.
- A solid understanding of physician practice protocols, policies, and operations.
- Specialty certification and at least 2 years of clinical experience, with nephrology experience highly preferred.
- Excellent written and verbal communication skills.
