California Licensed Ambulatory Clinical Pharmacist

A company is looking for an Ambulatory Clinical Pharmacist (CA License Required).

Key Responsibilities

Provide medication management, including reconciliation, dose optimization, and adherence counseling

Collaborate with the interdisciplinary care team to develop individualized care plans based on evidence-based guidelines

Monitor and evaluate medication regimens to ensure compliance with clinical criteria and therapeutic goals

Required Qualifications, Training, and Education

Doctor of Pharmacy (PharmD) and a licensed pharmacist in CA

Must meet two of the three criteria for advanced practice pharmacist status in CA

Completion of PGY1 / PGY2 residency is preferred but not required

1,500 hours of clinical experience under a collaborative practice agreement within the last 10 years is required

Strong clinical knowledge and expertise in disease state management and treatment guidelines
