Job Position: RN - Case Manager
Facility: Providence Health and Services
Location: Multiple Locations (Mission Hills, Torrance, Apple Valley)
Duration: 13 weeks contract with all possible extension
Shift: Day 5x8-Hour (08:00 - 16:30)
Guarantee Hours: 40 Hrs.
Orientation: 40 Hrs.
Charting System: EPIC

Pay rate: Blended Hourly Pay Rate (W2): $70.00 - $75.00 Blended Weekly Gross Pay Rate: $2,800.00 - $3000/wk
Job details:

• Years of experience REQ: 2-year acute care Hospital Case Manager experience performing Care Coordination and Discharge planning, HOSPITAL EXPERIENCE IS A MUST THEY NEED TO HAVE
• First-timers accepted: Yes - Clinical will screen 1st time traveller
• Weekend REQ: weekend rotation + meet dept needs
• CA state license: Yes
• Pending License accepted: Yes, but must be licensed upon start date
• RTO Restrictions: No more than 7 days
