Job Description:

EliteCare Medical Staffing is looking for a dedicated and experienced RN-Care Manager to join our Home Health team in Clearlake. In this role, you will be responsible for managing patient care in a home health setting, ensuring that patients receive high-quality care tailored to their individual needs.

Responsibilities:

• Coordinate and manage patient care plans in home health, providing comprehensive support to patients and their families.
• Conduct thorough assessments and evaluations of patients’ health status and needs.
• Collaborate with healthcare professionals to develop and implement effective care strategies.
• Educate patients and their families about healthcare management and available resources.
• Ensure compliance with all relevant regulations and standards for home health care.
• Travel to patients’ homes as required; mileage reimbursement will be provided at the government rate.

Qualifications:

• Current Registered Nurse (RN) license in the state of California.
• Home health experience is preferred.
• Must possess a clean driving record.
