Hospice RN

The Hospice RN Case Manager is responsible for providing compassionate, comprehensive end-of-life care to patients in their homes or care facilities. This role involves coordinating care plans, managing symptoms, and supporting patients and families through the hospice journey.

• Coordinate and manage care for hospice patients in collaboration with the interdisciplinary team.

• Perform initial and ongoing assessments of patients’ physical, emotional, and psychosocial needs.

• Develop and implement individualized care plans focused on comfort and quality of life.

• Administer medications and treatments to manage symptoms effectively.

• Educate patients and families on disease progression, symptom management, and hospice philosophy.

• Serve as a liaison between patients, families, physicians, and other care providers.

• Participate in on-call rotation for after-hours support.

• Document care accurately and timely in accordance with regulatory standards.

• Current RN license in TN (unencumbered).

• Minimum of 1 year of nursing experience (hospice or palliative care preferred).

• Valid driver's license, reliable transportation, and proof of insurance.

• Strong communication, organizational, and critical-thinking skills.

• Ability to work independently and manage a caseload effectively.

• Experience with EMR systems and interdisciplinary team collaboration
