About the Role To The Rescue is seeking a Registered Nurse (RN) - Home Health Case Manager to provide skilled nursing care and care coordination for adult patients in their homes. This role offers meaningful one-on-one patient care, professional autonomy, and a healthier work-life balance compared to hospital or facility-based nursing.
You'll manage patient care from admission through discharge, working collaboratively with physicians, therapists, patients, and families to help individuals remain safe, independent, and comfortable at home. We offer a supportive team environment, realistic expectations, and a $15,000 sign-on bonus for full-time RNs.
What You'll Do - Provide skilled nursing visits in accordance with physician orders and individualized plans of care
- Complete comprehensive patient assessments and ongoing evaluations
- Develop, implement, and update care plans in collaboration with the care team
- Coordinate services with physicians, therapists, patients, and caregivers
- Educate patients and families to support safety, independence, and health outcomes
- Document care accurately and timely according to agency and regulatory standards
- Communicate changes in patient condition to the appropriate team members
What We're Looking For - Active Registered Nurse (RN) license in the state of Iowa
- Graduate of an accredited school of nursing (ADN or BSN)
At least 1 year of nursing experience (home health experience preferred, not required) - Strong assessment, communication, and time management skills
- Ability to work independently while collaborating with a multidisciplinary team
- Valid driver's license and reliable vehicle
