About the Job:

Blize Healthcare is looking for a Certified Nursing Assistant / Home Health Aide to join our team. You will provide routine healthcare to patients at the patient's home or in a care facility in Solano County and Contra Costa County.

Responsibilities:

• Providing routine individualized care to the elderly, convalescents, or persons with disabilities.
• Monitoring and reporting changes in patient health status.
• Providing personal care, such as bathing and dressing patients.
• Provide specialized healthcare such as administering medication and changing bandages.
• Teaching family members ways to care for patients.

Required Skills & Qualifications:

• Positive and patient demeanor.
• Ability to build rapport with patients.
• Ability to handle physical workload.
• Experience in home health or hospice.
