Care Central Professionals Home Health is seeking a dedicated Occupational Therapist (OT) to join our compassionate and patient-focused team in Roseville, CA. • Conduct skilled OT evaluations and develop individualized care plans.

• Provide interventions to improve safety, independence, and quality of life.

• Educate patients, caregivers, and families on adaptive strategies, equipment use, and home safety.

• Collaborate with our interdisciplinary team to ensure holistic, patient-centered care.

• Maintain accurate, timely, and compliant documentation.

• Current California OT license in good standing.

• Graduate of an accredited Occupational Therapy program.

• At least 1 year of clinical experience (home health preferred).

• Excellent communication and organizational skills.

• Valid driver’s license and reliable transportation.

• Collaborate with interdisciplinary team members to ensure comprehensive care.

• Document all care provided in compliance with agency, state, and federal guidelines.

• Participate in care conferences and contribute to ongoing care planning.

• Current California OT license in good standing.

• Graduate of an accredited Occupational Therapy program.

• Minimum of 1 year of clinical experience (home health experience preferred).

• Strong communication and documentation skills.

• Ability to work independently and manage time effectively.

• Valid driver’s license and reliable transportation.
