Under the general supervision of the Nurse Manager or designee, performs diversified clinical duties in providing skilled nursing care to acutely ill or chronic patients in the home in any of the communities in the Home Care Division service area.

Job Description

Under the direction of the Clinical Manager, the Specialty RN will be involved in First Day Home visits,

Habilitation program assessments and visits, Hospital at Home scheduling and coordination and assistance with Home Health

Aide Supervisions and orientations

Competency in basic nursing skills. Excellent assessment skills. Ability to speak, read and write English.
