PRN Home health Wound Care RN/LVN

We're seeking an experienced PRN Wound Care Nurse (RN or LVN) to join our compassionate and skilled clinical team. This position provides in-home wound care and nursing services while ensuring the highest standards of clinical excellence and patient satisfaction.
Responsibilities

• Provide direct in-home wound care and clinical services in accordance with the individualized Plan of Care.
• Assess, plan, and implement wound care treatments while ensuring continuity and quality of care.
• Report changes in client condition promptly to the Director of Nursing and other appropriate personnel.
• Educate and support clients and families regarding wound care, treatments, expectations, and precautions-maintaining dignity and confidentiality at all times.
• Maintain accurate and timely clinical documentation in compliance with state, federal, and agency standards.
• Participate in agency quality improvement initiatives and remain current with best practices in wound management and home health care.

Requirements

• Current, unrestricted RN or LVN license in the State of Texas
• Minimum 3 years of nursing experience, including 2+ years in wound care
• Proven proficiency in wound care management, wound vacs, and dressing applications
• Ability to perform skilled nursing assessments and clinical documentation
• Reliable transportation with valid auto insurance
• Clean background check and criminal record
• Strong knowledge of HIPAA compliance and patient privacy standards
