Job Description
Registered Nurse Home Health Field RN providing Home Visits for Home Health Care.
Work Schedule: 5 days a week, 8-hour shifts (Monday to Friday).
Responsibilities Include:
- Utilizing the nursing process efficiently in patient care delivery.
- Assessing patient and family learning needs and implementing appropriate teaching strategies.
- Leading and collaborating with the nursing team and interdisciplinary teams.
- Using data and knowledge to evaluate and promote changes for optimal patient outcomes.
- Enhancing customer satisfaction, employee morale, and professionalism in nursing.
- Adapting care to meet the diverse needs of different patient populations, including respect for privacy and effective communication.
- Performing additional duties as required.

Physical and Environmental Demands:
This role requires physical capabilities including:
- Heavy Work: Lifting and moving objects weighing from 10 to 100 pounds.
- Regular exposure to potentially hazardous materials, including blood and body fluids, while maintaining safety and compliance standards.
Requirements:
- Must hold a valid Registered Nurse license in the respective state or a Compact License.
- Minimum of 1 year of RN experience; 2 years preferred.
- Open to local or travel candidates.
- Education: Required - Registered Nurse degree; Preferred - Associate or Bachelor's degree in Nursing.
- Valid Driver's License and Auto Insurance are required.
- Experience with HCHB EMR is ideal.
Certifications:
- Current Registered Nurse (RN) license in the state of practice.
- Current Basic Life Support (BLS) certification from the American Heart Association.

- Proficiency in using computers and web-based applications.
- Effective verbal and written communication skills.
- Strong interpersonal and organizational abilities.
- Ability to manage time effectively and work independently.
- Excellent judgment in clinical decision-making.
