Social Services Assistant
We are seeking a compassionate and dedicated Social Services Assistant to join our skilled nursing facility team. In this role, you will support the Social Services Director in providing high‑quality services that enhance the quality of life and well‑being of our residents. You will help plan, develop, organize, implement, and evaluate the social service needs of those we serve.
Your responsibilities will include:
• Assisting with planning and implementing a comprehensive Social Services program that provides counseling, support, and advocacy for residents and their legal representatives
• Identifying medical‑related social needs of residents and providing appropriate services to meet individual and group needs
• Maintaining accurate documentation related to residents’ social needs and care plans
• Consulting with the Director of Social Services and other departments on interdisciplinary needs
• Working directly with residents and legal representatives experiencing personal, social, or environmental challenges
Qualifications include:
• Bachelor’s degree in Social Work preferred; a bachelor’s degree in Human Services may be considered
• Previous experience in social services required; long‑term care experience strongly preferred
• Any state‑required certifications or licensures
